Collection
Procedures
Preparation
for Collection
- Positively
identity the donor to be tested.
- Explain
the test procedure to the donor. Provide donor instructions it desired.
- Confirm
that the donor has not had anything in his or her mouth for 10 minutes
prior to providing the oral fluid sample. If the donor has had anything
in his or her mouth within the last 10 minutes, wait 10 minutes more
to beginning the collection process.
The
Collection Process
- Inspect
the expiration date on the Intercept
collection device packaging and open the outer packaging containing
the collection pad and the specimen vial.
- Place
the specimen vial on a ftat surlace in view of the donor, instruct the
donor to grasp the handle and remove the collection device from the
packaging sleeve.
- Instruct
the donor to place the collection pad between the lower cheek and gums
and gently rub the pad back and forth along the gum line until the pad
is moist. Once moist, leave collection pad between cheek and gums for
a full two (2) minutes.
- Open the
specimen vial is in an upright position, with the tip pointed downward,
gently rocking the cap back and forth to avoid spilling the contents.
- After
two minutes, hand the donor the open specimen vial and instruct the
donor to push the collection pad into the specimen vial as far as it
will go.
- Instruct
the donor to snap the collection wand at the scored line against the
side of the vial. Instruct he donor in not tilt the vial or spill the
fluid inside.
Completing
the Process
- Hand the
specimen vial cap to the donor and instruct the donor to place the cap
onto the vial until it snaps, ensuring a secure fit.
- Instruct
donor to place tamper evident seal across the top of the specimen vial
and down the sides, date and initial.
- Place
the lntercept
specimen vial(s) and any required chain of custody form (see back) into
the specimen bag.
- Place
the specimen bag into the overnight shipping package and send the specimen
to the laboratory for testing.
Chain
of Custody
- Positively
identity the donor to be tested.
- Complete
the collector (green) section on the chain of custody form.
- Explain
the test procedure to the donor. Provide donor instructions it desired.
NOTE:
Confirm that the donor has not had anything in his or her mouth for
10 minutes prior to providing the oral fluid sample. If the donor has
had anything in his or her mouth within the last 10 minutes, wait 10
minutes more to beginning the collection process.
- Inspect
the expiration date on the Intercept
collection device packaging and open the outer packaging containing
the collection pad and the specimen vial.
- Instruct
donor to place the tamper evident seal from the chain of custody form
across the top of the specimen vial and down the sides. Have the donor
date and initial the seal.
- Instruct
the donor to complete the donor information (beige) section and donor
affidavit on the chain of custody form, verifying the signature and
date.
- Complete
the collector affidavit on the bottom (blue) section of the chain of
custody form and provide the donor with his/her copy of the form.
- When split
specimen is required, use a second device and have the donor place the
second collection pad on the opposite side of the mouth.
- Place
the Intercept specimen vial(s) and Copy 1 of the completed chain
of custody form into the specimen bag and initial the bag.
- Place
the specimen bag into the Airborne Express LabPack and send the specimen
to LabOne for testing, utilizing the pre-printed address label provided.
- Distribute
the additional copies of the completed chain of custody form to the
appropriate parties.
